PROGRAM MANAGER

Major Duties:

The Mental Health/TBI and DD program Manager's managerial duties and responsibilities involve significant recommendations/decisions, which have a direct and substantial effect on the delivery of Mental Health/TBI and DD services throughout its facilities, and related programs. Candidates must able to manage other staff, able to work 40-45 hours per week, able to pick up staff/clients’ phone calls anytime in an emergency situation, able to write effective programs and practice an effective crisis resolution for our clients.

Coordinate the delivery of quality client’s care by assuring the care plans of direct support staff are complete, accurate, and followed. Be responsible for the performance of and/or oversight of critical client’s care activities, including intake, physician and case manager’s communications. Provide support for the client in day-to-day program.

Key Requirements:

  • BS or Master's in a health related field with licensed.
  • Demonstrated leadership experience in a multifaceted health care system.

PROGRAM COORDINATOR

Major Duties:

Program Coordinator for a caseload of people with Mental Health, TBI, and developmental disabilities with psychiatric and/or behavior disorders. Program Coordinator is responsible for the oversight of clients at Relieve Care homes to promote their well being, self sufficiency goals, and therapeutic care, maintain relationships, and supervise team development.

Key Requirements:

  • BS in a health related field or a licensed social worker.
  • Professional counselor with some experience.

HOUSE COORDINATOR

Major Duties:

The Relieve Care House Coordinator provides quality healthcare, directs Activities of Daily Living, and ensures continual progress with Clients toward their Care Plan and goals for developing Independent Living Skills. The successful House Coordinator sets the standard to leads, manages, directs, communicates, improves and maximizes the quality of life for clients and assigned team members. The House Coordinator maintains and cherishes the dignity in individuals of all ages.

It is the goal of Relieve Care, and the House Coordinators to assist the Clients and Family members to meet and exceed their defined goals, and assist families, professional services and governmental agencies to provide in the best interests of the Clients

Key Requirements:

  • High School / GED. Experience in health related field.

DIRECT SUPPORT STAFF / CNA

Major Duties:

Provides direct care to clients following the individual service plan, treating each client with respect and dignity, recognizing individual needs and encouraging independence. Fosters privacy and independent throughout. Must be compassionate, reliable and motivated candidates

Key Requirements:

  • High school diploma or general education degree (GED).
  • Three to six months related experience preferred.

ADMINISTRATIVE ASSISTANT

Major Duties:

Performs a variety of office job including researching functions and uses word processing, spreadsheets and company software. Duties also include filing and data entry. May assist with overflow work from administrative and executive assistants, and fill in for the office duties as needed.
